These curves show a flat lateral sliding movement over time i.e. a “plateau”. It is actually supposed to drop vertically to a minimum acceptable level which it clearly does not.

Where is the opportunity in this? The window of opportunity is that the system can now be easily corrected at less cost than someone just trying to fix the symptom.

What causes the symptom then? Executive Managers is mostly expected to think linearly as shown below:

Linear Safety Objective

The above graph shows that fatalities can be reduced by linearly spending more and more on corrective actions. However, the above graph implies that either everybody can die today or everybody will be safe tomorrow – which is obviously wrong.

The answer is to use the following graph:

Non Linear Safety Objective

The above graph shows that NOT everybody is going to have a fatality today, nor is everybody going to be safe. The acceptable cost per fatality can now be optimized at the lower end of the curve.
